How do I help my concentration, sleep, and procrastination problems?

I’m really bad at taking tests, my grades are good(5 A’s, 2 B’s, taking honors classes too :D ), but my tests could be better.

So after analyzing my self and thinking what is holding me back from doing excellent on tests, I found out that it is from:
1. Lack of Concetration during a lecture.
2. Sleepyness in Class
3. Procrastination at home

So how can I help these three things get better?

How can I concentrate more?

How can I not be sleepy, besides from getting 8 hours of sleep?

How do I help my procrastination problems at home?

These are all related because since I procrastinate and stay up late doing homework, I don’t get enough sleep, now since I’m sleepy, I always lose concentration during a lecture during class. Now since I lost concentration, I don’t get the material, which thus causes me to do not so great on tests.

    What are the consequences of staying up for 24 hours? Sleep problems?

    I’m currently working on a summer assignment, and I am halfway done with it (thank god!). I want to enjoy my summer while it lasts, as well as have "fun" with my project time by staying up really late. If i stay up late, then I’d probably be done with my project as well as kick my sleep habits back to normal. By that I mean, I’ve been staying up till 5AM lately (and of course wake up 8 hrs later, which would be a little past noon) and since school is starting really soon, I want to wake up and sleep at a "normal" time. I was thinking that maybe if I stay up long, I could tire myself out for the day so I would be forced by my body naturally to sleep. Do you think this would work for kicking my sleep habits back to normal? Also, what are the consequences of staying up for 24 hours?

    What can I do about sleep problems?

    I used to have trouble falling and staying asleep. Recently I had surgery to correct a deviated septum so that cured the sleep apnea. I no longer have trouble staying asleep, but I still have trouble falling asleep. What can I do about this? Should I see a doctor, a psychiatrist, what?

    How can I sleep with breathing problems?

    I know this is my third question but new details. (:
    The only sleep I got today was probably from 10AM-1:30 PM
    I don’t get much regular sleep (i often go to sleep as late as 5 or 6 AM and sleep to afternoon). I really want to sleep tonight but I have anxiety-related breathing problems that make falling asleep hard and uncomfortable. Any tips?
    forgot a detail. my mom and i already went to a clinic where we had pills perscribed. they do not seem to have taken effect.
